Issue of birth certificate

If any one can help me.!!
My Fathers was born in 1956 and his middle name had been corrected in 2010
My father middle Name name has been changed from "ANTONIO" to "ANTHONY" as per the ghazette copy published in mumbai in 2010
In this regard i have recieved a Ghazette copy of 2010 stating that his middle name has been changed from ANTONIO to ANTHONY.e
I had tried applying for a fresh copy of my fathers birth certificate in the brihanmumbai mahanagarpali but they failed to issue me a Certificate with the Change in the midle name. (ANTHONY). in addition to that i have submitted attested copies of his passport driving licence etc which states his Middle name as "ANTHONY"
is there any other procedure i have missed out so as to reflect the change in name on his birth certificate.

Tell me whether your father is still alive or not , if he is alive then he can himself make a supportive affidavit to confirm the fact or otherwise you on behalf of your fathr can do de same.
