Mother name changes in my birth certificate

Sir my name is umesh gaur and my DOB is 18th Feb 1994 and my birth certificate also of the same year but on that time in my birth certificate by mistakenly my mother name listed as Geeta which actually not her name in other document.Actually her name is Pushpa Devi which need to be change in my birth certificate to make my passport.
Please do some needful help to solve this problem.

Consult the birth certificate issuing authority to know the procedure of changing the name, complete the procedure and get it rectified.
If not possible, your mother can give a sworn affidavit regarding both are her names.

In the recent new rules birth certificate is not mandatory for passport. You can provide other documents for the purpose.

Submit affidavit in the Passport Department stating that both name is of the one and the same person.

There is lot of gap between the names stated by you, how such an error has occurred at the time of your birth?
Was it not known/checked at the time of issuance of birth certificate, if the story is correct?
File a suit for declaration through a local lawyer, if your statement is correct.
