From where to get affidavit for name change? It is a stamp paper? On what conditions does notary attests affidavit? Who can be witness? Further procedures etc.?

For Assume title or change of name procedure you may declare/sworn affidavit before the Judicial Magistrate court and after that publish in Gazette and or vernacular news paper of your state.

Affidavit, Govt. Gazette notification and addition you have to insert advt. in local news papers.

So it means after getting affidavit attested i have to send it controler of publication delhi. And thats it? Is there any condition on which notary attests such affidavit? Who can be witness?
