Procedure to withdraw consumer case

Respected Lawyers,
I need your valuable guidance in withdrawing the case from Hon'able District Consumer Forum Gautam Buddha Nagar, U.P.
I had filed the case on a mobile company for faulty handset. Now the opposite party is ready to refund the amount.
Can you please guide me, what is the procedure to withdraw the consumer case?
Also what are the key points to keep in mind while withdrawing the case?
If possible please share Sample Application Letter for case withdrawal.

Appear before the district consumer disputes redressal commission on date fixed for proceeding and submit that the opposite party has replaced faulty hand set and I wish to withdraw the complaint. The commission shall record your statement and dismiss the case as withdrawn. If you wish to withdraw earlier move an application stating the facts and move to the concerned commission.
