Regarding having full name of the mother printed in birth certificate

Dear Sir/Madam,
My name is TN Deepak Bharadwaj, my wife's name is Archana and my son D. Akshay Kannan's birth date is 25/Jan/2013 (in SP Hospital Adambakkam Chennai). In my son's birth certificate, mother's name is printed as Archana Deepak which I want to get it changed as Archana Deepak Bharadwaj for applying his passport. My wife's name in her savings account in State Bank of Travancore is Archana Deepak Bharadwaj and I can produce the bank passbook as proof. What else do I need to submit to get this changed in the corporation office and what is the general procedure been followed for the same?

You can apply for the correction of name along with supporting document to the Corporation of Chennai.

mostly Passport officials asking gazette notification, so you can apply modification by way of paper publication or get new birth certificate with proper name.

The passport agent said that even if the mother's name in the birth certificate is printed as D. Archana instead of Archana Deepak it is fine rather than getting it changed as Archana Deepak Bharadwaj, so I hope there shouldn't be any difficulty in getting this done. Please advise me on this.

In my opinion, the Corporation authorities do not have power to change the name already registered with them at the time of submitting application for birth which contained entire details including the one which now sought to be changed. It can be changed only through a court order, please check it up with the authorities for confirmation. Instead my suggestion is that you may submit Annexure "H" declaration along with the passport application which will suffice your requirement.
