Is there any specific RTI format in Registrar of Company at Mumbai office? if it is. please share the link.

Just list your points. Now a days there is limitation to number of questions that can be asked in single RTI app. Better check up frorm office oron website where you have to deposit

You can use the normal RTI application format to seek information from R.O.C. office .

In RTI information basically one has to give his details and contact address
His relationship to details asked for if applicable
Case reference
point wise queries in short including copy of any data /document required
The prescribed RTI application fee
Once copies are ready department will take payment as per number of copies.

Persona information of any one and classified documents cant be answered.
