There is a transfer of flat from my mother name to my name(son) with Gift deed, now society demands Apendix 20 (1,2) 21/22 and 23 along with gift deed,
is it mandatory?

What are the contents of said Appendix. If procedure exist, should submit.

Can submit copy of gift deed, in case of need may present original for comparison.

Contents of appendix are required to be perused, however, it is a procedural query which can be sorted out by author and management.
