Specimen affidavit for name transfer.

my neighbour has purchased the flat in mumbai suburb area, but Electricity and Munipality Property Tax still stand previous name, and now both the authority want to produce Name transfer affidavit by purchaser, so i request the member can i get Name Transfer Specimen Affidavit from this forum.

its simple just mention details of flat purchased by you . that flat was standing in name of X . that society has approved application for transfer of flat . enclose NOC certificate from society . if necessary ask the power distribution company for sample format
