eventhough the act does not specify any format, many states have designed so format.

I personally sent few RTI letters to a public sector bank and I never sent the same on any set proforma. It can be entertained even if the application is written ordinarily.

rest of the information you can seek from bank or from Bank's website.

1. There is no specific format, infact no format can be prepared. Since, the RTI can be utilised for many type of informations.
2. So use plain paper. The fee (Stamp duty) is Rs.10/- which can be paid to the treasury, IPO, Court fee stamp, etc. Out of which, I feel the court fee stamp is more convenient which is available with all stamp vendors.
