Wife's name change in my daughters bc

Sir/Madam,
I am from bangalore. In my daughter's birth certificate my wife name updated as Belcy Dharmarajan, but in her passport her name is Jaya Raja Belcy. We want to apply passport for my daughter and we need to have the correct mother's name in birth certificate.
We went to bangalore BBMP office in Upparpet and they told to file a notary for the name change. Is is required as there is no change in the name only in the Birth certifcate the original name to be updated.
Please guide me the procedure. Thanks in advance

Contact local lawyer, they would guide you. It is a very small few hours affair.

Yes! It would be better for you to take help of a local lawyer for preparing notarized affidavit.

Submitting the documents including preparation of notorised affidavit will hardly a job 3-4 hours. Department may take 3-4 days.

@Sudhir Kumar sir: In Hospital they have updated my wife's name as Belcy Dharmarajan and the same has been added to my daughter's BC. Initially we didn't notice and recently we came to know the change in mothers name.

if you are unable to find out lawyer nearby, you may go to the stamp vender for purchasing the stamp paper for affidavit and he would advise about the typist or document writer who will prepare and get it notorised. You may go to BBMP office and complete the formality.

@Sudhir Kumar Sir - Should I need to approach any other office apart from BBMP, Upparpet. They are telling 100 reasons for the name change (hope they are expecting some money). What else can I do sir?
