Are you talking about affidavit in chief?
If yes then kindly ask your lawyer he would assist you showing the format.

Here all your verbal evidence needs to put into writing.
The documents(primary evidence) needs to be exhibited at the time of tendering the affidavit.

You have left us to guess ?

A documentary evidence is always in writing.

When we agree in life some thing orally and then choose to execute in writing either under compulsion of law or on own option, then it becomes documentary evidence.

Otherwise when we have to give some evidence in courts,or forums,on oath the same may be either recorded by court or the court may ask you to submit it in writing form called AFFIDAVIT .

It is not so easy to draft an affidavit in chief unless you see any of its copy filed any other case.

It would e better if you take help ofn advocate at this stage or ask for a copy of the same filed in any other case.
The official of the Forum may help yu.
