I, full name, Hindu, adult, Indian Inhabitant, resiiding at..........having occupation as do hereby state on solemn affirmation that :

1     My maiden name is Ms************************** and I got married on __________ to Mr._____________ as per Hindu Vedic Rites and Rituals.

I say that my name is changed after marriage as Mrs.**********************.   I say that for the purposes, I am going to use the said name henceforth.

2   I say that this affidavit is required to be submitted before Govt authority forthe purpose of changing name.
