I would be very glad, if i would come to know the procedure to change the name in the BIRTH CERTIFICATE as per the PRESENT NAME. We are in an intention to change the name.
I would request you to post the procedure / format.

Contact the Registrar of Deaths and Births of your District for getting the detail of formalities to be completed.Normally an affidavit is to be filed for change of name.
