Gratuity withdrawal form

I have completed 5 years in one company and resigned it. After that I have joined new company..Which

form should I fill up to receive Gratuity money from my first company. They have provided me "Form-F" Is it

correct form or should I fill up "Form I" to get the money in Cash?

Please assist....
