It is better for you to approach there at the office personally, purchse the application form, fill up the form, attach all the required documents/certificates, Fees/DD etc., and submit in the office on the same day.
But, dear Rakhi, please donot forget to keep all the required documents/certificates, (6) Passport size Photos of yours and Amount of fee while appraoching there.

dear u could check the list of lawyers from chd...every one would ve helped u.
i do practice in chd.....incidently i left chd yesterday for 5 days vacations in Jalandhar, as high court is closed till 6th......u r falling short of time better u visit chd...not far from delhi.
Commission Office and Bus Stand both r in Sector 17....u can board Haryana Roadways volvo to chd.
the form in fact is in Hindi...for gen category it is for Rs. 500/- only.
