How to make annexure k affidavit

Dear Friends,

my friend want to make an annexure K affidavit . Anybody please tell me what is the procedure for that.

If it is signed by notary , Passport office will accept that one?.. how long will take to make annexure K affidavit?

Hi Can anyone let me the know the procedure of the same.I have to apply for passport renewal and wanted to remove the spouse name but still the divorce case is not resolved kindly someone tell the procedure how to get the Annexure K.

This is what i am goin to do..as i will be following annex K. Make a affidavit as per annexure K and keep a copy of divorce proceeding/ not necessarily decreee, and any proceeding like petition...and present it to the passport office when asked. should not be a problem
