Name change document

Hi,

My mother wanted to get her passport made. She submitted all documents needed. The acknowledgement letter issued by passport office says that documents verified with originals, however confirmation from issuing authority is required. My mom's maiden name was VK Thambai which was changed to Nalini Bhaskaran Poduval after marriage. The name change was notified in the gazette in 1975-76 (date dont remember) She is a retired government officer and had submitted all documents including an affidavit regarding the name change. However, passport office insists on the gazette notification or newspaper notification. Now, something published in gazette in 1975-76 is difficult to trace. What is the solution? One more problem is that her school leaving certificate is in the form of booklet wherein everywhere its mentioned VK Thambai but accept on page number 3 which unfortunately happens to be the main page of the booklet wherein it inadvertently mentions her maiden name as V K THABAI instead of V K THAMBAI. What is the solution in both the above situations.

You swear any affadvit drafted by advocate and get attested by District Magistrate/Sub divisional Magistrate submit the same to Passport Office. If they do not accept you have to  file suit in the local court and obtain decree for all these changes in name.

Most govt library keep old papers you can enquire there it may prove fruitful if you remember the year or date
