specific format of affidavit

Sir,
Will you please supply me specific format of affidavit given by presedent/Secretary at the time of registeration of society in Punjab.Also intimate me what is the provision of society act if out of seven memebers three members belongs to same family or related with each other(Blood relative)

Societies Act does not put any embargo upon the relatives being the office bearers. If all the members choose to change them it could be done by a duly conducted election.
