This is Mr Avinash V Shrotriya from Chinchwadgaon-Pune 411033 8888824764, we got passport in Feb.2011 in Mumbai when we were living in Govt Quarters at Bandra, That time permanent address was given of Pune-411 033 and present address was of Mumbai. Since Last Jun. we shifted to Pune. Our Passport have only Mumbai address, as there is no provision of present and Permanent address in the Passport, had it been so then no problem but since that is not the case to change the address in present passport means to apply and get new Passport. My daughter who is a student is more in need of address change than others. It is learnt that supporting documents as address proof needs to be in the name of applicant, since the applicant is member of the family documents are in the name of parents/Flat owner so how the documents be in the name of children in the family. Moreover they still insist for Ration Card as an Address Proof where as it is clearly mentioned in the Ration Card that it is not to be used and accepted as an address proof, since it is for getting grossery under Public Distribution System. Hence in a given situation what is the solution for the same

as per Passport act you have to submit all requirment mentioned below:
Address Change
What are the documents required to be submitted for a change of address?
Form-2 duly filled-in.
Proof of Address (any of the documents listed below is acceptable)
NO police verification is required for change of address.]
Proof of Address (attach two of the following documents)
1. Applicant’s ration card
2. Letter from a reputed company on their letter head regarding stay proof;
3. Statement of running bank account along with a letter from the Bank
4. Election ID card
5. Letter from the concerned Administrative Department (in the case of Govt. Employees)
regarding the applicant’s present address as per departmental records
6. Appointment letter of reputed companies
7. Income tax assessment order
8. Spouse’s passport copy
9. Parents’ passport copy in case of minor
10. Domicile certificate issued by Tahsildar
11. Water bill /telephone (landline) bill /electricity bill / Gas connection confirmation
12. Property Tax in the name of the applicant
13. Lease agreement with additional proof such as cellphone bill.

Thanks for reply sir however my basic questions are
1) When permanent address of Pune was given while applying (Jan.'11) for passport, it was issued on (then) present address of Mumbai, so what can be done in this regard now ?
2) Required documents will be in the name of parents/Head of the family, who is Flat Owner and not in the name of children who are student and not minor.
3) When recently shifted to permanent address how we can submit last year's Telephone bill and if it is of private Telecom Co. will it be acceptable ?
4) Why permanent Driving license having photo as well as address in plastic card format is not acceptable where as Ration Card, which is not a valid address proof but issued for food items to issued under Public Distribution System ( For white Card this is also not applicable )A note to such effect is printed on every Ration Card, in spite of this, even Govt agencies insist for Ration Card, as a Valid Address Proof, at the most with some other address proof.
If children/applicant/s of Passport, don't have such documents, but heir parents have and Ration Card cancellation slip issued in May'11 of parents indicating relation of applicant with parents, having required documents, will be acceptable,in lieu of Ration Card ?
