Would anybody furnish the copy of application for registration for adoption of a child and its procedures and list of documents to be filed and the authority to file etc.

Adoption deed duly signed by both spouse supported with the proof of the residence, birth certificate of the child are the documents which are required to be produced before the registrar. Simple application mentioning your desire and prayer is to be made and there is no specific performa for this simple cause.
