under what conditions adverse entries in acr are proved to be biased? Also under what conditions an enquiry on adverse entries will be called a biased?

When the entries are either not shown or are recorded adverse to the factual position then such entries of ACR are called as biased and if the entries are make correct and duly communicated to the concerned employee within time even then enquiry is made and wrong finding is given then such enquiry is biased.
