Regarding the first query it is advisable to contact the local muncipality/hospital where your daughter was born. In absence of details it is not possible to give the rules governing such local authority.
Yes you can apply for PAN card for your 2 year old daughter. After she attains majority she can apply for rectification of details and she shall continue holding the old PAN number.

You can obtain any number of copies from the office of the Registrar of Births and Deaths located in the local ward of municipality/corporation of your area where your daughter took birth. Make an application and remit required fee, you shall be able to get the original certificates.
Regarding PAN card, do you really need to obtain a PAN card for your 2 year old daughter? As a guardian, you may transact all the business including financial affairs for your self on behalf of your minor daughter till she attains the age of majority. PAN card may serve no purpose at present.
