Change of name in passport

This query is : Resolved
The name present in my passport is SHAAD SHAKEEL AHMED s/o SHAKEEL AHMED,
but i want to change it to SHAAD SHAKEEL AHMED MANIYAR s/o SHAKEEL AHMED MANIYAR due to my surname MANIYAR being present on all of my marks card and degrees.
please tell me the procedure to rectify this,
I will greatly be thankful to you if i get some information or some contact of someone who can help me in this matter.

Contact passport office of the local area.now a days TCF is appointed as an franchise for verification of documents.Those people will verify all your documents along with police report of the local area,your application will be forwarded and you will get name changed passport.

Change of Name
What are the documents required to be submitted for a change of name?
In addition, depending on the category below applicable, there are additional requirements, as indicated against each:
(a) NORMAL CHANGE IN NAME:
i) Furnish original newspaper cuttings giving change of name published in two newspapers – one in English
and the other in a local language.
ii) Furnish an affidavit as per format given in Annexure ‘E’ of the instruction attached to passport application.)
(b) FOR CHANGE OF NAME AFTER MARRIAGE:
(Change of surname, while retaining the given name)
i) Two copies of Marriage Certificate issued by the Registrar of Marriages
(OR)
ii) A notarised joint affidavit from the husband and wife.
Note: If unable to produce a joint affidavit, furnish an affidavit with wedding photograph, wedding invitation card
and a letter giving reasons for not being able to give joint affidavit. Marriage certificate issued by Church and Qazi’s Certificate (with a certified translation) in case of Muslims can also be given as proof of marriage. In all cases, a
copy of the husband's passport should be submitted, and the original passport produced for verification at
the counter.
(c) CHANGE IN NAME ON ACCOUNT OF DIVORCE: (Surname change)
i) Divorce Deed duly authenticated by the Court
ii) Deed poll affidavit (as per the format given in Annexure ‘E’ of the instructions attached to passport application.)
(d) CHANGE IN NAME IN CASE OF REMARRIED PERSON (Surname change)
(AFTER DIVORCE OR DEATH OF SPOUSE)
i) Divorce Deed in support of applicant’s Divorce, or Death Certificate of the previous husband / wife, as the case
may be.
ii) Copy of marriage certificate (Original to be produced) issued by the Registrar of marriages in respect of recent marriage. OR Joint affidavit from the husband and wife
The US Embassy/Consulate has asked me to separately indicate my surname on the Passport. What do I do?
Form No.2 (Miscellaneous Services) is to be filled-in and submitted. An endorsment in Passport made with
the first-name and surname separately indicated. The fee applicable is Rs. 300/- and Passport delivery will be made on same day after 4:30 PM.

Mr. Nadeem has reproduced the instructions contained in the passport application form. Comply with the appropriate requirements so that ur name can be got corrected in the passport.

first you must correction and published with your area Gazette office press and pursuant to publish to famous news paper then you will submit before the concern officer.
