There is no process in law as far change of name of a trust is concerned in A P. You need to pass the necessary resolutions and convey your intention to the Registrar. Only on confirmation of the registrar the name change can take place. Once this is done then all statutory organisations like Income tax, bank authorities etc...which are connected with the trust must be informed about such name change.

Where is the need of format for the resolution? It is very simple decision of the Trustees of the Trust vide which on a specific date they intend to change the name of the trust from one name to other and for that purpose they authorise any one of them to contact to the office of Registrar.
