Dear Sirs,
I worked as a manager with a textile manufacturer in Bangalore from November 2005 to May 2007, resigned and then started my own freelance career. I was a member of the EPF/FP and the employer was depositing my PF/FP contributions to the respective EPFO office in that area. Due to my own fault (call it procrastination), I did not claim my accumulated amount till 2015.
I did not have my PF account number. Therefore I submitted my claim forms duly filled in to my employer's office requesting them to provide the account number and forward the documents to the EPFO in June 2015. But the employer's HR department misplaced the papers. I again resubmitted a fresh application in September 2015. But came to know that the company was closed by then.
Since then I have been following up with them and as advised by their MD I again couriered a fresh application in December 2015 but did not receive any reply. I have been running from pillar to post but since all their staff had left, I tried to take help from a few of their ex HR staff who tried to help me but without success. I am a senior citizen living in Kolkata and I have nobody to turn to for help. Hence I have decided to visit Bangalore next week and contact the Area or Regional office of the PF commissioner for help. But I do not have my provident fund account with me.
Respected sirs, can you please advise me how can I get back my PF money? All I have are my appointment letter and approx. date of leaving the service. Shall be grateful to know the procedure. Looking forward to your kind response,

you surely have the name of the company and the employee number and atleast one payslip/one IT Return of the relevant period.
file an RTI to the concerned EPFO office to give you the EPF statement furnished by the company during the period you were an employee and also for a year or two beyond as a cautious approach. The statement ought to contain details of employee number, pf account number of employee and amount to be deposited in his account. That will give you the necessary information and you can recover the money lying in the account

you need not visit Bangalore.
Since company is closed you need not get attestation from company.

Dear Shri Nagamani Sir,
I have been able to get a reply from the RTI giving my account detail. Meantime the employer surfaced and he signed my PF claim application. Problem is, my PF account name was recorded as Arun P. Chattopadhyay while my application bears my full name, i.e. Arun Pratap Chattopadhyay. My bank account submitted also bears my full name, i.e. Arun Pratap Chattopadhyay.
Now the PF officer has given a form to get my full name endorsed by the employer saying this is the same person. It is a simple case but the employer has again gone missing and we are unable to contact him! The PF officer is refusing to accept the application without the endorsement. I am at loss, and tired of running from pillar to post like this. Is there a simpler way to sort this out? Seek your kind suggestion, please..

PS: Basically, what I have now at hand..
1. Completed PF claim settlement application form, signed by Employer with enclosures.
2. Declaration explaining so much delay in submitting the claim
3. RTI reply giving my complete account statement.
The only discrepancy is, account name is Arun P. Chattopadhyay and my application and my bank account bear my full name, Arun Pratap Chattopadhyay. This shortening was done by HR of the company as my personal records including the PAN card bear my FULL NAME.
