I am aware it is fundamental right of citizens to change from one religion to another and there is no limit on changing religions from one to another. If one converts from Hindu to Sikh religion, what written legal "proof" is needed that person is now a Sikh? Is a certificate/letter of conversion written by Gurudwara sufficient legal proof? Or person has to get it registered, or do some kind of legal affidavit for future possible legal requirements.
There is no registration procedure , a letter issued by the concerned Gurudwara is sufficient in my opinion. After conversion if you want to get change your caste in your school records you will have to file a suit for declaration and injunction following the procedure laid down in u/s 80 of CPC

Is there any need mutation for changing in the certificates, the letter from the upcoming religion is necessary, otherwise not

For conversion of religion no certificate is required. Mere declaration in this regard is sufficient.
