If I can get some inputs on following:
what are the additional documents to be compulsorily submitted with passport application by a married woman i.e. the documents which are not required to be attahced by others.
Is the submisison of marriage certificate compulsory for a woman applicant??
Is declaration in Annexure D applicable in the cases when the pre marriage surname of husband and wife are same??

Dear Purohit
No additional documents are to be submitted apart from what is generally submitted viz.
While applying for a fresh passport attach two copies of the following documents:
(a) Proof of address (attach one of the following):
Applicant’s ration card, certificate from Employer of reputed companies on letter head, water /telephone /electricity bill/statement of running bank account/Income Tax Assessment Order /Election Commission ID card, Gas connection Bill, Spouse’s passport copy, parent’s passport copy in case of minors. (NOTE: If any applicant submits only ration card as proof of address, it should be accompanied by one more proof of address out of the above categories).
(b) Proof of Date of Birth (attach one of the following):
Birth certificate issued by a Municipal Authority or district office of the Registrar of Births & Deaths;
Date of birth certificate from the school last attended by the applicant or any other recognized educational institution; or an Affidavit sworn before a Magistrate/Notary stating date/place of birth as per the specimen in ANNEXURE ‘A’ by illiterate or semi-illiterate applicants.
N.B.: In the case of applicants born on or after 26.01.89, only Birth Certificate issued by the Municipal Authority or the Office of the Registrar of Births & Deaths is acceptable.
(c) Citizenship Document if applicant is a citizen of India by Registration or Naturalization.
(d) Government/Public Sector/Statutory body employees should submit “Identity Certificate” in original (ANNEXURE B) along with Standard Affidavit Annexure I.
(e) If the applicant is eligible for “ECNR” attach attested copy of supporting document (see COLUMN 15 of the INSTRUCTIONS AND GUIDELINES FOR FILLING UP THE APPLICATION FORM).
(f) If the applicant was repatriated at Government cost, enclose documents to show that the expenditure, if any, incurred by the Government of India on his/her repatriation has been fully refunded to the Government of India, Ministry of External Affairs.
(g) If the applicant was ever deported to India, give details of Emergency Certificate/Passport.
Submission of Marriage certificate is compulsory not only for a woman applicant but also for a male applicant.
Your third query is confusing how come the pre marriage surnames of husband and wife be same. Clarify what you meant or what the query is?

i must suggest u pls visit nearby General Post Office....first time forms for PP can be submitted there as well...u ll get sufficient help from there as well.

Dear experts,
Thanks for the fast & detailed input and also for the practical tip of visiting the GPO.
Clarifictaion regarding third querry is :
my wife's maiden name was Anita purohit and my name is Surendra Purohit. Due to this, there is no change in name/surname of my wife after maraige. This is common in our community. Querry is : in such a case is the annexure D essential??
I have visited the GPO and got the advise that Annexure D is essential in this case and marriage certificate is not required.
I have gone through the list of docs required to be submitted ( as provided by you in first mail and also on the nic web site) but could not find the mention of marriage certificate as essential docs for passport. U r also confirming that marriage certificate is compulsory for passport for both man and woman applicant.
I am just curious to understand : depsite of not being listed in govt web site in lthe list of docs how come the marriage certificate has become essential doc?? any deatils.
