Upsc attestation form- declaration of drunk and drive fine

Sir,
I have been convicted for drunk and drive in 2019. I have recently applied for UPSC civil services. Do in need to declare Yes in the attestation form when asked whether i am fined by a court of law. I have a general order copy downloaded from the court website which does not mention my name. My name also has a minor spelling mistake in the place of the respondent. I am unable to get any other records related to the case at this point. The order reads as following : Accused present. Adv. for
accused filed application u/Sec. of
Cr.P.C.
The substance of the
accusation read over and explained
to the accused and he pleaded guilty
voluntarily.
The accused is convicted for
the offences and sentenced to pay
fine.
I.D.S.I for a period of 30 days.

If i have to declare Yes in the attestation form, what records can i attach. If i do not declare, will it show up in criminal antecedent verification.
