Almost all kinds of formats of Affidavits/Undertakings and guidelines are available in the websites of Passport Office. However, there is absolutely no guidelines and/or formats for obtaining a Passport for MENTALLY CHALLENGED/RETARDED DEPENDENT. Therefore, can any one in this Club having knowledge on this issue throw some light please.
