you will have to get name of your mother corrected in your birth certificate . approach Registrar of Births for correction in your mother name in birth certificate
T. Kalaiselvan, Advocate (Expert) 09 June 2014
If your mother's name was error in the birth certificate to that of other relevant certificate, you can make a general affidavit stating that both the names are same and belong to the same person but erroneously recorded, you can attach the relevant documents denoting your mother's both the names as evidence to your affidavit, you can convince the passport officer in personal interview, if it is not successful, you may approach a lawyer and get the things corrected by due process of law.

File a civil suit inserting therein the school and the board as defendant and obtain the desired relief but prior notice is must.
