Name of first class magistrate

I had my signature change affidavit done. The office that needs the affidavit wants the name and address of the First Class Magistrate on the affidavit. Is it possible?

Probably the office demands verification/attestation done from the office of a Magistrate.

You may take help of some local lawyer to find out the exact process for the said purpose.
