What is the format of memo which is submitted in court while giving some details about bank account, etc. Is it verified and whether it is accompanied with an affidavit ?

it is given in an application format itself and annexe proof of the details also (like pas book etc.)

It is better to submit Affidavit with particulars & supporting documents on the Subject Account and submit the Affidavit along with a formal memo -
