Hello Sir ,
I am resident of palwal (Haryana )
My name in 10th , 12th mark sheet and in ration card is written as “ Vijay Kumar” .
My surname is Goyal but it is not written in any documents.
When I came to Gujarat for doing my undergraduate and post graduate studies my name is written as “ Kumar Vijay Ghanshyam” due to addition of father’s name with your name in Gujarat .
Now I want to generalized osteoporosis to USA for further studies , for which I am going to apply for passport . Whenever I Fill any form I have to leave column of surname as blank. I want to add my surname in my name before applying for passport.
Is it relevant to change my name at this stage ?
What is the proper procedure for changing / correction of name at this stage.
Till 12th In my documents my name is “ Vijay Kumar” and after 12th all my degrees have “ Kumar Vijay Ghanshyam” as my name .
What is the procedure to obtain a document that states that “ Vijay Kumar” , “ Kumar Vijay Ghanshyam” and “ Vijay Kumar Goyal” are name of a same person ??
Are these documents are internationally accepted ??
Kindly guide me about it.
