NCDRC good draft for early hearing petition

Please give us a good draft format for early hearing petition to be filed by a 81 year old complainent in NCDRC. The case is of year 2004, The complainent won from both district consumer redressal form and From State commission. Now the case is pending in NCDRC since 2010. This year in march the revision petition was admitted and case has been pending in "List in Due course"
