She should apply in the organization of her husband's employer.

PLEASE SEE THE PENSION PAYMENT ORDER
It is in the form of book, some times it is called as pension book.
In the first page, the amount of family pension and the person who is eligible to receive family pension, etc would have been mentioned.
Now, what the wife has to do is to apply to the pension payment officer or the organisation where her husband worked stating the fact of death of pensioner enclosing the death certificate, copy of pension book, etc.
After, she has to follow the official regularly.

In normal course, Pension Payment Order (PPO) or the Pension Book, itself, contains the necessary santion of the competent authority about the basic amount of pension, as permissible to the widow on death of the pensioner. The only formality is required is necessary order for the payment of family pension by the pension paying authority in favour of the widow on death of the pensioner. For that purpose, some simple formality is required. The widow will have to send an application with intimation of date of death of the pensioner along with his death certificat for the purpose of issue of necessary sanction by the concerned authority.
Address of the pension paying authority is very much mentioned in the PPO book with official stamp of the authority. So, the widow need to apply to that authority only.
