Need sample format of amended ws

Hello,

my Amendment to WS application is passed in court order and now i have to file the amended changes in WS.
Currently dont have a lawyer and search is on. But in meanwhile can someone please give me a sample format document to showcase how a passed amendment is filed.

I though we need to rewrite the WS with additional points but a friend suggested its not like that but only additional points need to be written. Will be really helpful if i can get a format sample.

I took advise of 2 lawyers one advised to create a new copy of WS and underline amended lines. Other recommended to modify same WS with red strike for deleting a content and sticking paper cheat for new additions.

Which method shall i follow? Thanks.
