Inclusion of surname

I do not have a surname. I want to include a surname for myself and for my family members (wife and two children). How should I go about it ?

Make an affidavit and then publish a gazette notification.
Meet a local lawyer to complete this.

Thanks Mr Devajyoti for your advise. Can you suggest some lawyer at Chennai to help me out. I meet couple of them at Velachery and most of them said that they do not undertake this work.
