Change of dob in pan

I want to apply for pan card for my father. The original has been lost and we want to apply for a new one. Also the details like DOB,correction in name and address needs to be corrected but I have only Aadhar card as proof of identitiy on his behalf. He is handicapped and cannot walk. I want to get all of his documents of identity. Please let me know how can I proceed.

What is the reason for change in DOB?
what proof you have regarding his DOB now to be inserted?
You can apply online at NSDL site, either send documents i.e. aadhar, DOB proof, photo, residential proofs through speed post or upload scanned copy of given size on line. Pay fee online.
