It is very interesting to note that format on civil procedure code has been sought by you in criminal section.

Sir, There is no specific format rather it is very simple to draft any legal mandatory notice under section 80 CPC. You provide the facts of your matter so that its rough draft may be submitted to you for your correction and further despatch to the addressees.

You can get the format in 'filing procedure' by Arulselvam (Malathi Publications).
