I am facing a problem to make an afidavit for following subject:

How the name in passport appeared wrongly/ Whether it is mistake of passport office or from your side? Do you have required documents in support of your correct name?

If it is mistake of passport office, it would be rectified easily.

For affidavit drafting get the services of some lawyer r if to be submitted to passport office get a draft from them.

So, by an affidavit you can get your name changed in the passport. Contact a Notary Public in the local who will be able to draft, prepare and sign the affirmation made in the affidavit at a nominal cost.
