Execution Petition by self

Dear Experts, I am a retired Mumbai based person and want to file Execution Application / Petition which I am told I can make on my own as per format provided by the experts in this forum. I have completed the format for your further guidance to correct the text as I can't afford the fancy amount asked by local advocate who had obtained a Consent Term Order charging a hefty fee. I hope you will understand my situation and help me file this petition on my own. I shall attach the Exhibits and copy of court order under Consent Terms with the petition. Regards,
