please contact the passport office from where the original passport was issued along with available proofs stated.
sent a registered letter to them asking for the remedies.
wait for 2 to 3 weeks for repliy.
if no positive response come,sent a RTI query on the subject.
beware that you have not done any crime. even if the date of birth is erroneously mentioned in the application,it is the duty of the passport authorities to show the correct date based on the relevant documents submitted along with the application.

Dear Navin,
It is not understood, how the Passport could be issued by the Passport Authority without verifying the original documents indicating your date of birth when all other documents, and importantly the school certificate bear different date of birth.
However, in your case you will have to apply to the respective Passport office for rectification of error.
In case of Change of Address, Name, Date of Birth, Place of Birth, Appearance, Spouse Name, Name of Parents/Legal Guardian, a fee of Rs. 1,000/- is prescribed. In such cases a fresh Passport is issued by the authorities. There is no need to file any writ petition in any court of law for this purpose.

You send a letter to the Passport Authorities, along with the proofs. It is the mistake of the passport department, they should have checked the documents.
Normally they will issue a fresh copy after rectifying it.
