Hon'ble members, Is there any provision in IT Act regarding submission of information on 'tax evasion to IT authorities'?

There is no provision in the Act about it. Nor there is any format it prescribed formally or informally.

If you have to give any such information it is advisable to send it to either

Director of Income-tax (Investigation) or
Director General of Income-tax (Investigation) of your area/ region.

There is no need to give either your name or address.
