Claim amount not paid

Dear All

i got one detail from client who having problem with general insureance company.His daughter admited in hospital after discharge he has submitted all original bills and necessary documents time is atmost over more than 3 month but still compnay are not responding so please suggest in which language i have to send notice if possible please give me some basic format of notice

Format of notice are not supplied on this site and the language is your option though it should either of the Indian languages.
