Birth place and place to which you belong

Hello sir,
I am filling an attestation form of a govt. organization as I have been selected in it.
I belong to SC community.

1)It asks for Birth Place
2)Place to which I belong
3)Place to which my father basically belongs.

My Queestions:
Q1) Sir,Can Place 2 and Place 3 be different?
Q2)Sir, Is it necessary that place 2 and the place from where my caste certificate has been issued to be identical? or if they are in different districts but in same state than it is ok?

Please clear my doubt

How it is different from "place of ordinarily residence"?

Hypothetically I would like to fill up the 3 points as follows giving two scenario. By going through it, you will appreciate the real meaning of the query.

First Secnario
1. Lucknow
2. Lucknow
3. Lahore (now in pakistan)- - now settled in Lucknow

Second Secnario
1. Kanpur
2. Lucknow

Many persons place of birth is different from domicile and domicile of parents may be further away.

I was thinking that "place to which a person belongs" and "place of ordinarily residence" are identical.
