SIR

A LADY WANT TO CHANGE THE NAME OF HER MINOR SON( 3 YR) IN BIRTH CERTIFICATE TO GET ADMISSION WITH NEW NAME IN SCHOOL.

HER SON OLD NAME IS USED AS A NOMINEE IN HER LIFE COVER POLICIES ONLY. BUT NOW SHE WANT TO CHANGE HER SON NAME.
