Esic card not prepare

Dear Sir,
I am working in limited co.my hr esic deducted from my salary since my joining date(July 14) but they not prepare my esic card with my dependant family member.In this situation my mother suffers from some disease but i have not esic card or number .Kindly suggest me what i do this.

Write to Head-Hr,appointing authority, Md,Chairman under proper acknowledgment with a copy to you...............and announce that your mother needs to be treated and advance should be issued to you Rs.............for treatment in ESIC empaneled hospital e.g..............and estimate of hospital for Rs.............is attached.
If mother's sickness can't wait........ Get your mother treated and submit all bills for reimbursement.
You can also lodge complaint with jurisdictional ESIC office/Inspector,o/o Labor Commissioner,Inspector appointed under Shops & Estbs Act etc.........and even employee's/Trade unions

Ask your HR to get the needful done. May meet Seniors and can request in writing with acknowledgement due.
