The land is my grandfahter(died) name, so we want to trasfer the name from grandfather to my father . Land tax paid Receipts in the name of My father. so ask who are the appeal authority and any format for this transfer

This form is not required to be with you rather it is a revenue department format means copy of its register. You have to just move your application supported with affidavits enclosing the death certificate of your father and rest of the proceeding is to be initiated by revenue officers.
