Two names of a person

(Querist) 26 March 2013 This query is : Resolved 
Dear Sir,
My father had two names, on his all legal documents he has his official name, but on his death certificate he has his nick name.
The hospital is refusing to change the stating that we told them this name and it is not their mistake, Municipal Corporation is saying that they need correction letter from hospital.

Now can I get an Certificate from court stating both name belonged to same person, so as to present it to PF office & LIC.

You or any legal heir of your father can affirm an AFFIDAVIT before the Judicial Magistrate that two name are in separate documents which is one and same identical person.You can file a Petition by approaching to the Magistrate that an order may be passed by directing to the Municipality and Hospital for necessary correction.You can also publish the same in a local Newspaper.

After serving a legal notice, you can definitely file a civil suit for declaration and further injunction against the authorities you mentioned in the query.

File application in the CJM Court for issuance of correct certificate under Death and Birth Registration Act.
