Regarding birth certificate

I was born on 1977 my parents have not registered my birth in municipal corporation. The hospital in which i was born is closed. I was born in jamnagar (gujarat) & i am leaveing in jamnagar.
How to obtain my birth certificate from municipal corporation.

1-Apply for Birth Certificate with your date of Birth before Municipal Corporation
2- The Municipal Corporation Health Department will issue Birth Certificate - if it is already entered with them - if not entered - they will issue no entry Certificate -
3-Apply before the Competent Authority/ RDO /Divisional Magistrate/ for an order to Registrar Births & deaths to enter your Birth event in their records .
4-There is delay in entering your birth event for which an order is required to Registrar of Births & Deaths
5- After entering your Date of birth as per the orders mentioned above the Registrar of Births Will issue Birth Certificate to you .
6- This is the procedure in AP ,with some variation, the same procedure will apply in Gujarath State also.
